release films and/or reports to physicians and/or patients, we require a signed release form.
In

Release
of X-Ray Films and/or Reports : This form is to release any films taken at Southwest Diagnostic Imaging Center to the patient, physician, or another imaging center.
Authorization for Release of X-Ray Films, Reports, and Medical Records: This form is to release any films taken at another imaging center to Southwest Diagnostic Imaging Center for comparison to previous treatment/surgery/effectiveness or as comparison to a recent procedure.
